He didn't take them off - as Crouch Jr. transformed from Moody to himself, the extra limbs merely popped off.

Unless you are questioning whether Crouch Jr. had to take the fake leg and eye off of the captured Moody to complete his disguise, rather than the Polyjuice potion doing it for him. In which the answer is yes, since the captured Moody possessed neither in his trunk-prison.
