It's true that he does ask you that and that his Raticate does disappear, whether it died or not or if you caused the death is never confirmed. I always assumed it died because of all the doping Blue was giving it. How else did he get a lv16 Raticate?

All the other text based stuff is true, the parents one is implied since he has a sister and a grandfather, but no parents are ever mentioned.

 

A quick Google search says it is fake.

I think I recognize those pokéballs they have clipped on. Might have been a BK or McDonalds Kids Meal toy from the better part of five or ten years ago. The only thing throwing me off is that I don't think they had the black stripe as a divider, but that could just be hand-painted.
